Personal Information Collection Statement
Please provide the personal data as requested and ensure that it is complete and accurate. Some of the personal data requested is required for the stated purpose but some may be voluntary. Voluntary data will be specified in the collection form as appropriate.
The data collected will be used for retail and goods delivery or other directly related purpose(s) and will be retained for a period of 12 months or until the relevant use of the requested data has been completed, whichever is later.
Your personal data held by our Unit will be kept confidential, but it may be shared with our customer service staff of the University and other parties, including administrative staff, for the purpose(s) stated above.
Under the Personal Data (Privacy) Ordinance of Hong Kong, you have the right to request access to and correction of your personal data held by our Unit. Requests for data access by filling out a Data Access Request Form OPS003 (revised 09/2012) specified by the Privacy Commissioner for Personal Data, and follow-up data correction requests can be made in writing to Ms. Janice Kwok of CLNC via email hkbucmclinic@hkbu.edu.hk or by post to Room 305, 3F, Jockey Club School of Chinese Medicine Building, 7 Baptist University Road, Kowloon Tong, Kowloon. The University may charge you a fee to cover the administrative cost for data access, while data correction requests will be fulfilled without a fee.
